Should men under 50 be screened for prostate cancer—and if so, how?

This study looked at over 500 men aged 45–50 with elevated PSA levels and compared two methods of prostate biopsy: MRI-targeted and traditional systematic biopsy. Results showed that using MRI-guided biopsy in addition to systematic biopsy provided the greatest benefit.

So MRI works best when combined with systematic biopsy—especially in younger men.

WOMEN’S YOUROLOGY

Can pelvic floor exercises really beat technology when it comes to vaginal laxity?

This study compares pelvic floor muscle training (PFMT) to radiofrequency (RF) treatments in women experiencing vaginal looseness, a concern reported by up to 38% of women. After just 30 days, both groups showed improvements in sexual function, urinary symptoms, and pelvic floor strength. But here’s the catch: after 6 months, the women doing PFMT had better pelvic floor contraction and overall outcomes than those who had RF treatments.

It shows that consistent pelvic floor training—not just tech-based procedures—can lead to longer-lasting improvements in sexual health, bladder control, and vaginal support. If you're dealing with vaginal laxity, this is a strong case for trying PFMT first.

This week, someone asked me, “What would be the best thing too do to increase your girth and length permanent for your penis? With out the pill scams.”. The most reliable and safest option backed by science for increasing length, and to a smaller extent girth, is using a medical-grade penile traction device. These devices gently and consistently stretch the tissue over several months, with studies showing a modest but real gain in length (about 1–2 centimeters if used as recommended). If you're curious about how these devices work and which ones are popular, I’ve reviewed several in a YouTube video about traction devices, where you'll also find links to the ones I mention.

For girth specifically, hyaluronic acid fillers are being used more often and can offer some temporary increase, but they come with potential risks and require repeat treatments since they aren’t permanent. Surgical options can lead to more dramatic changes but also bring the highest risks for complications and disappointment, so they’re only worth considering for very select cases. Honestly, before getting too caught up in all these options, I’d urge you to focus on your overall health—losing excess weight, eating well, and exercising can make a real difference in how you feel about yourself and actually improve the appearance of your penis.

  • It’s normal for erections at night to decrease a bit as you age, but you should still notice them well into later years.

  • If you suddenly stop getting them, it could be your body’s early warning sign—time to check on things like testosterone, exercise, stress, sleep, and general health.
